Driving in Italian hill towns is not for the faint of heart. Pictured below: squeezing a minivan down a narrow lane....

... which is at least better than squeezing up a narrow lane--  remember that most vehicles in Europe have standard transmissions.  If you want an automatic, make sure to state this when you book your car.

If you're staying more than 15 days, your best rental option is probably a short-term "lease-purchase" from Renault or Peugeot. The "lease-purchase" is a formality that allows the car to be "owned" by you, so that it can then be resold to Europeans minus a new-car tax. Our experience with Renault  was first-rate. You can easily book automatic transmissions, too.

Note: in Italy, you are required to purchase Collision Damage insurance, even if normally your credit card covers this type of insurance.  (The Renault and Peugeot deals include all coverage with  "zero-deductible".)
